#WVGS19 Workshop Highlight Moving Toward an Age-Friendly Health System

The West Virginia Geriatrics Society will host a day of intriguing and insightful sessions and workshops at the 2019 Annual Geriatrics Summit. Don’t miss the first of those sessions, “Moving Toward an Age-Friendly Health System” presented by Mark Newbrough at 9:30 AM.

Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI) in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA). The goal of the initiative is to rapidly spread the 4Ms Framework to 20 percent of US hospitals and medical practices by 2020.

An Age-Friendly Health System is one in which every older adult’s care:

Is guided by an essential set of evidence-based practices (the 4Ms)
Causes no harms, and
Is Consistent with What Matters to the older adult and their family
After the session, attendees will be able to:
Describe the Age-Friendly Health System initiative
List and describe the 4M’s of an Age-Friendly Health System
Describe strategies for beginning the Age-Friendly Health System work in their own health systems.
